ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM

In consideration of being granted permission to participate in Escape Artistz, I, on behalf of myself, my family members, my executors, heirs, successors, and assigns (including any minor children listed below), hereby assume all the risks associated with my participation. I also agree to hold harmless the owners, managers, contractors, agents, and operating licensees of Escape Artistz, as well as their respective employees, agents, and authorized third parties (referred to collectively as the “Released Parties”), from any claims that may arise due to my participation in Escape Artistz, as follows:

A. WAIVER, RELEASE, AND DISCHARGE: I waive, release, and discharge the Released Parties from any and all liability, including but not limited to liability arising from the negligence or fault of the Released Parties, for any personal injury, disability, property damage, property theft, or any actions of any kind that may occur to me, including those related to my travel to and from Escape Artistz.

B. INDEMNIFICATION: I indemnify, hold harmless, and promise not to sue the Released Parties for any liabilities or claims arising from my participation at Escape Artistz.

I hereby certify that I am physically fit for participation in Escape Artistz and that there are no health-related reasons or problems that would prevent my participation in Escape Artistz.

I understand that this Accident Waiver and Release of Liability Form will be relied upon by the Released Parties and will govern my actions and responsibilities at Escape Artistz.

I acknowledge that the management reserves the right to request my departure at any time.

I understand that the Released Parties are not responsible for errors, omissions, acts, or failures to act by any other party or entity conducting a specific event or activity in connection with Escape Artistz.

I acknowledge that my participation in Escape Artistz may include activities that test a person’s physical and mental limits, and that it may carry the potential for death, serious injury, and property loss.

I understand that I may be photographed, videotaped, or audio recorded during my participation in Escape Artistz. I hereby grant permission for my photo, video, audio recording, or likeness to be used for legitimate purposes by the Released Parties or any of their licensees or assigns.

I acknowledge the confidential nature of Escape Artistz and agree not to take photos, videos, or audio recordings of my participation.

I understand that by providing my email address, I will be subscribed to Escape Artistz’s mailing list and will receive emails about new rooms and promotions. I acknowledge that Escape Artistz will not sell my email address or send spam.

This Accident Waiver and Release of Liability Form will be interpreted broadly to provide a release and waiver to the maximum extent permissible under the applicable laws of the jurisdiction in which Escape Artistz is located. The laws of the jurisdiction in which Escape Artistz is located shall govern and be the place of venue for any legal action.

I hereby certify that I have read this document and fully understand its contents.

I am aware that this is a waiver and release of liability, as well as a contract, and I sign it of my own free will. (If under 18 years old, a parent or guardian must sign.)
